It operates Medicare Advantage Institutional Special Needs Plans (I-SNP and IE-SNP) and High Needs ACO REACH designed to improve care for Medicare’s beneficiaries who reside in long-term care facilities or assisted living facilities who need the type of care that is provided in a nursing home.
We are RAC-CT RN’s that are also reimbursement specialists. We will work to improve your facilities CMI for Missouri Medicaid and PDPM through the MDS process .
